To print a full size submission form, you
must have Adobe Acrobat Reader installed. Follow the steps at
to install Adobe Acrobat Reader.
Click on the icon below to open the submission
form in Adobe Acrobat Reader. If you have problems opening the file, you
may need to upgrade your web browser and Adobe Acrobat Reader to the latest
version.
Note: Your browser's security settings may
prohibit you to open the file directly from the internet. Save the file
to your hard drive and open the saved file in Acrobat Reader to overcome
this problem.
Select Print from the File menu. Make sure
your printer is set up correctly for A4 pages. Print the form on a single
A4 paper (black and white)
For future use, you can save the form on your
hard drive to print it without accessing our web site (Select Save from
the File menu)
