TABLE : A GRADING SYSTEM FOR LYMPHOPLASMACYTIC GASTROENTERITIS:

DESCRIPTION

GRADE DESCRIPTION OF RELEVANT LESIONS

NORMAL

0-2

LAMINA PROPRIA
Lymphocytes and plasma cells either present focally affecting 25 - 75% of glands or diffusely with less than 3-4 cells in array between glands
SUPERFICIAL EPITHELIUM
Normal with infrequent intraepithelial lymphocytes
CRYPT VILLUS UNIT
Normal
MISCELLANEOUS
No fibrosis or oedema present

MILD 2.5-3

LAMINA PROPRIA
Mild diffuse to moderate focal lymphoplasmacytic infiltrates; 5 - 8 cells in array between glands
SUPERFICIAL EPITHELIUM
Normal columnar epithelium with mild numbers of intraepithelial lymphocytes
CRYPT VILLUS UNIT
Normal
MISCELLANEOUS
Mild oedema and/or fibrosis

MODERATE

3.5-4

LAMINA PROPRIA
Moderate diffuse to severe focal lymphoplasmacytic infiltrates; > 8 cells in array between glands
SUPERFICIAL EPITHELIUM
Flattening of the absorptive epithelial cells
Moderate numbers of  intraepithelial lymphocytes
Decreased numbers of goblet cells
Multifocal erosions
CRYPT VILLUS UNIT
Slight blunting of the villi
Slight decreased numbers of crypts and glands
Separation of glands or crypts by moderate cell infiltrates and/or fibrosis
MISCELLANEOUS
Moderate oedema and fibrosis

SEVERE

4.5-5

LAMINA PROPRIA
Severe diffuse  lymphoplasmacytic infiltrates; > 8 cells in array between glands
SUPERFICIAL EPITHELIUM
Cuboidal absorptive epithelial cells
Moderate numbers of  intraepithelial lymphocytes
Marked decrease in numbers of goblet cells
Multifocal to coalescing erosions/ulcerations
CRYPT VILLUS UNIT
Fusion and blunting of  villi
Lymphangiectasia
Partial to complete obliteration of  glands and crypts by cell infiltrates and/or fibrosis
MISCELLANEOUS
Severe oedema and fibrosis
